Insurance claim for Personal Injury through HAW 2025

Initial Action

  • Ensure that an Incident Report is completed as soon after injury as possible.
  • Obtain and follow proper medical advice as soon as practically possible.

Making a Claim

Insurance is via Hockey Australia so their website is the starting point. If you google
“Hockey Australia insurance” you will see reference to a hockey specific website which
takes you to the broker, Marsh. Look for the Hockey Australia Insurance program, then
“Make a Claim”

This takes you to au.sportscover.com. You need to complete a Personal Accident Claim Form
Request and send it asap (time limits apply- 30 days from accident).

You will then be sent an actual Claim form which requires more details such as accident
report, witness statements, a medical (or dental diagnosis) linking the damage to the hockey
injury, employment details if you lost time from work etc. There is a time limit of 120 days
for lodging this. Next, your claim should be accepted and you will be sent more information. 

Benefits Payable

If the treatment is covered by Medicare or private health insurance, it’s not covered by this
insurance and neither is the gap. Neither public hospital costs are not covered, nor
anesthetist fees, MRI scans and X-rays.

Dental (to sound whole teeth only), physiotherapy, chiropractic, ambulance, private hospital
accommodation (if no private health cover) are covered, but with provisos.

Physiotherapy is paid at 95% less excess of $100 for first 5 visits, 80% for next 5 visits and
75% thereafter.

You can recover 75% of your medical, dental and physio expenses apart from a $100 excess,
but maximum payment is $3,500.

If you missed time from work, you may be entitled to 80% of net income lost, to a maximum
of $350 pw.

In the case of death or permanent disability, a lump sum up to $250,000 can be paid.
Student Allowance, Domestic home help, Rehabilitation, Injury assistance and bed care
benefit are all available where criteria is met, together with other benefits under Clause 4.4 of
the Policy.

There are lots of exclusions set out in Clause 6 of the Policy including pregnancy.

Payment is made at the end of your treatment up to a maximum of 12 months from injury.
You need to keep all receipts and submit them when the work is finished, within 12
months. Payment ceases if you leave Australia.

Benefits cease if you resume playing hockey or even return to training so you’ll be out until
the treatment is completed.
